Forgot Your Password?

 

Your ProntoForms username and password are used to login to both the mobile application and the ProntoForms portal.  These are set by your team administrator, Prontoforms support, or your wireless provider when your user account was created.  View instructions on changing your password here.

Note: Support cannot change, reset, or provide your user account password.

If you have lost or forgotten your username or password, there are several ways to recover your account information. 

  • On the App: From the ProntoForms Mobile login screen, click the Forgot Password? link (located below the Login to ProntoForms button).
