User Getting "Access Forbidden / Invalid Credentials" After Successful Login to the App on a Galaxy S5

I have a Mobile-only user that has started receiving a "Access Forbidden / Invalid Credentials" inside the app after successful login.  From what I can tell, nothing about his account has changed in the past 4 days.  He shows a last reconcile date of 04/22/16, but doesn't show any date/time value under Last Login.  The user is accessing the app via a Galaxy S5.  Has anyone seen this message before?

    Hi Karen, I have escalated this as a ticket to our support team, who will be best able to assist you.

    The "Last login date" actually only reflects logins to the web portal, not the mobile app.  Keep in mind that a "reconcile" is what pushes submitted forms from the device to the ProntoForms portal.
